Our community has been at work developing Transition strategies for children, youth and adults with special needs. Research has supported that planned transitions will maximize the opportunity for those children, youth and young adults to access and obtain seamless services to meet their health, educational, vocational and social needs from a variety of service providers within the community.
The information provided on this web site is intended to provide a variety of definitions, resources, tools and sources of information to help individuals find the right mix of supports throughout life’s transitions
