Referrals

  • In order to access a service or agency, you must make a referral. Here is a list of services you can access according to your child’s age.

0 – School Entry

Below is a list of agencies that your child may access at this age depending on their needs. Keeping in mind that waiting lists exist, the referral should be made as soon as possible.

With a doctor’s referral, specialized assessments may be accessed through pediatric centers and hospitals including

*Referrals for assessments should be initiated by the age of 2 years to accommodate wait lists and ensure proper transitioning for school entry.

*Entry to school is very important. Some of the agencies listed above, can assist the family with transitioning to school at the time of your child’s registration. Please access education for more information.

Junior Kindergarten – High School

At this time, your child may require updated assessments. Below is a list of agencies that your child may access at this age depending on their needs. If you are not already involved with an agency, please keep in mind that waiting lists exist and a referral should be made as soon as possible.

*Transition between schools may occur at this age. Some of the agencies listed above, can assist the family with a transition to a new school. Please access education for more  information.

High School – Adulthood

All assessments should be completed between 16-17 years. Assessments will be required to access services and supports. Please refer to the assessments section for the types of assessments available. Applications for the following should be completed prior to 18th birthday:
